Fees for Medical Nutritional Therapy

 

Visits are billed in 15-minute time slots. All initial visits are for a minimum of 30 minutes, but generally take a full hour. Most follow up appointments are for 30 minutes or an hour. You may follow up as often or as little as you need.

 

  • 15 Minutes=$20.00
  • 30 Minutes=$40.00
  • 45 Minutes=$60.00
  • 60 Minutes=$80.00

 

Fees for Diabetes Self-Management Education

 

Classes range from 2-3 hours each for a total of 8-10 hours over   3-4 group classes

 

  • 30 Minutes=$31.00 Individual
  • 30 Minutes=$20.00 Group

 

Insurance Plan Information

 

Each insurance plan will vary in coverage for Medical Nutrition Therapy (MNT). Coverage will be different, even within the same insurance company from plan to plan, and from state to state. You should contact your insurance company, to determine what is covered. Be sure to check your deductibles. Katherine Shaw is a participating provider with Medicare, John Deere, Cigna And United Health Care (this does not necessarily mean that your visit will be covered).

 

Many insurances consider Diabetes Self-Management Education(DSME) and Medical Nutrition Therapy (MNT) separate things. Real Life Nutrition is now offering a Real Diabetes Management Program (DSMT). Medicare, Aetna, United Healthcare, and Cigna are currently covered for some plans.
